Investigation Summary
At about 7:15 a.m. on June 29, 1994, Employee #1, of Acromil Corp., was working on Cincinnati 5-axes, hydrotel machine #15. He used a portable ladder to climb to the top of the machine, which was 7 to 8 ft above the ground. He removed the top guard from the timing belt and drive (3 in. wide). At this time he was standing on a control box, attached to the side of the machine. As he climbed on the top of the machine, he grabbed the V-belt with his right hand by mistake. As he pulled the belt, the machine turned itself on and his right index finger was amputated in the belt and pulley.
