Investigation Summary
On March 6, 1993, at about 2:30 p.m. at the job site of Massie Roofing Co., Pasadena, California, Employee #1 fell off a 14 foot 8 inch high roof. The employer is a roofing contractor. Employee #1 had been working with the employer for 1 year and 9 months as a roofing crew member. On the day of the accident, their crew reroofed the building at that job site. After installing the paper over the applied tar on the reroofed surface, they had to press the paper so that it would stick to the applied tar evenly. At the east side of the roof, Employee #1 had his left foot on top of the parapet and with his right foot tried to press the paper over the tar. In the process, his left foot somehow got stuck in the tar on top of the parapet. He then had his right foot also on top of the parapet in order to have a good force to pull his left foot off the tar. As he pulled his left foot, it abruptly released and he lost his balance, falling off to his left side. In his fall, he became entangled with the electric service before hitting the ground. He stayed 5 days in Huntington Memorial Hospital and was treated for hand fractures and a forehead wound. He underwent surgery on March 7, 1993. The employer was cited for violations of T8CCR3203(a)(3) general and T8CCR1670(a) serious.
