AT 11:45 A.M. ON SEPTEMBER 23, 1993, EMPLOYEE #1 WAS FEEDING THE PAGES OF A BOOK INTO A CUTTING PRESS. AS HE WAS ADJUSTING THE PAGES, HIS HAND SLIPPED INTO THE PRESS AND TWO FINGERS WERE CRUSHED. HE WAS TAKEN TO POMONA VALLEY HOSPITAL, TREATED, AND RELEASED.
