Investigation Summary
On March 30, 1995, Employee #1, of California Steel & Tube, was assigned to help #8 stand roll forming machine stack materials. He overheard a conversation between the foreman and the roller forming machine operator that the products were coming out bad due to the dirt. The machine operator had asked Employee #1 to clean the in-running rolls by dipping the rags in the cleaning solvent and allowing them to drip into the in-running rolls as part of the production process by the #8 stand roll forming machine. Employee #1 noticed that the products were still coming out bad. As he then cleaned the in-running side of the rolls, his right hand became caught by the rollers, cutting his right middle finger and partially amputating his right index finger. The employer was cited with a total of six citations; five are serious violations and three of the five are serious accident-related violations.
