Employee's hand lacerated on diamond grinder

At about 2:05 p.m. on August 25, 1994, Employee #1 was working on a Co-Burn, 112-E generator (lens grinder). She had not worked on that machine for 5 to 6 months, but was helping another operator finish a job. Employee #1 mounted the lens on the chuck and adjusted the power of the lens. She was adjusting the thickness of the lens with her right hand while her left hand was still on the lens. Somehow the chuck moved and pulled her left hand into the diamond grinder. Employee #1 suffered a severe laceration on her left hand. After the investigation, the employer was cited for the serious violation of T8CCR 4184(a). Other violations were found and the employer was cited accordingly.
