EMPLOYEE SUSTAINED CONCUSSION DUE TO FALL FROM SCAFFOLD

At approximately 9:15 a.m., on August 15, 1995, an employee was plastering the side of a one story building on a scaffold 11 ft from the ground. While he was working, the employee's feet became entangled in hoses and ropes that he had on the scaffolding work surface. He tripped and fell to the ground. He suffered multiple bruises, a concussion, and was hospitalized.
