Investigation Summary
At about 8:45 a.m. on October 12, 1992, Employee #1, of Ioptex Research, Inc., working alone, had climbed to an attic on a seven-step metal ladder to measure the support beams and was ready to descend. He placed one foot on the top of the ladder but the other foot was caught on something in the attic. He lost his footing and fell approximately 8 feet, bruising both legs. Employee #1 was treated and released from the medical clinic that day.
