Investigation Summary
At about 11:00 a.m. on September 4, 1992, Employee #1, of Wadman Corporation, was decking the roof of a commercial warehouse under construction in Glendora, CA. He and a coworker were putting a ledger angle on a south wall of the skeleton steel structure. Employee #1 was moving the ledger when he stepped off the deck to a truss and lost his footing. He fell through an open space between the roof trusses and the ledger angles, about 20 ft to a cement floor, and sustained head and facial injuries. He was not wearing a safety belt and lanyard. His wife said that he had not been given a hard hat or safety belt to use, and that he had not been wearing any body protection at the time of the accident.
