Employee burned when flammable liquid catches fire

On October 19, 1993, Employee #1 was in the paint mixing room at Advanced Packaging and Products Co., a paint manufacturer, dispensing toluene from a wall-mounted Morrison nozzle into an open 1 gallon plastic bucket. The toluene vapors ignited and toluene spilled out of the bucket. The flames spread to Employee #1's pants legs. His pants were made of 35 percent cotton and 65 percent polyester. Employee #1 suffered third-degree burns to both legs. The contributing factors to his injury were the use of an open bucket to transport a flammable liquid, and Employee #1's lack of flame-retardent clothing. The suspected ignition source is static electricity.
