Investigation Summary
At approximately 7:50 p.m. on November 7, 1994, Employee #1, of BMS Textiles, Inc., Gardena, CA, was retrieving a box of yarn from the warehouse. The stored materials were not stable or secured. Employee #1 fell approximately 6 1/2 feet when the string holding the boxes broke. Employee #1 sustained a broken left wrist and right knee and was taken to Gardena Memorial Hospital, Gardena, CA, for treatment and hospitalization.
