Investigation Summary
Employee #1 was working on a sawdust bagger when the machine jammed and stopped operating. He climbed into an open bin at the top of the machine; in the bin there were exposed auger bits. While Employee #1 was clearing the blocked augers, a coworker hit the ON switch, activating the augers and breaking Employee #1's left hand in two places.
