Employee's finger lacerated when caught in tapping machine

Employee #1, of Holland & Herring Manufacturing, Pomona, CA, placed a part into a tooling fixture in a Dumor tapping machine, serial #8255.2201. He used his left index finger to hold the part in place while activating the hold-down clamp and turn-on switch with his right hand. Employee #1 forgot to remove his left hand and his index finger became caught by tap #1 of the two taps and was pushed into the part and fixture.
