Employee's fingers amputated in bakery machine

At approximately 1:00 p.m. on April 22, 1996, Employee #1, an apprentice engineer at Pacific Pride Baking Company, was servicing a machine in the packing area when his right hand became caught in the chain and sprocket drive. His right middle and ring fingers were amputated. Employee #1 had not locked out the power supply prior to working on the machine. In a subsequent interview, he stated that he was not supposed to lock out the power supply when the machine was in production.
