Investigation Summary

Investigation Nr: 201794450
Event: 04/09/2001
Employee injured when boxes fell during unloading of freight

At 7:15 a.m. on April 9, 2001, Employee #1 of Sierra West Express suffered a back injury after a load of boxes weighing approximately 1600 pounds fell on his back during the unloading of freight inside a trailer parked in the dock area of the company in Commerce, California. The accident occurred inside of a JB Hunt trailer, Serial Number 211091, while it was being unloaded at the Sierra West Express terminal. The fork lift driver was using Fork Lift Number 17 for unloading the freight with the help of Employee #1. The driver stated during the interview that he had warned Employee #1 not to stand near the raised forks during unloading, but that Employee #1 did it on his own and got hurt when the load shifted and fell from a height of about 6 feet. When the driver put his forks under a pallet to lift it from the top of a stack inside trailer, the load shifted and fell on Employee #1 as he was standing by and helping in unloading. Employee #1 suffered a 6-inch-long laceration to his back in that accident, requiring a stay in excess of 24 hours at a local hospital.
