Investigation Summary
At approximately 12:30 p.m. on January 30, 2001, Employee #1 was loading and tying portable toilets onto a flatbed truck for delivery when he tripped and fell off of the right side of the truck. The flat bed is 96 in. wide by 24 ft long plus the tail gate. Employee #1 was securing a portable toilet called a half unit, which is on wheels. The half unit is 57 in. by 29 in., which leaves a walkway on the flatbed of about 20 in. on each side of the half unit. Employee #1 was feeding a rope through a loop on the top of the half unit when he fell about 50 in. Employee #1 sustained a cracked lumbar disk, cracked left hip, and his left wrist was broken in two places. Employee #1 was hospitalized for four days.
