Employee injured by fall from ladder on movie set

At 12:30 p.m. on April 16, 2001, Employee #1 was injured after falling from a ladder from the second floor to the first floor surface. At the time of the accident, employees were involved in the construction of a movie set. Employee #1 and a coworker were working on the second floor of the set at the height of approximately 9 feet. They were installing an overhead "T" bar to support false ceiling tiles. The distance between the "T" bar and the floor of the second floor was approximately 8 feet. To reach working level, the employees were working from the second or third step of a six-foot step ladder. There were three employees working together installing the "T" bar. A coworker and Employee #1 were working across from each other, each installing one end of the "T" bar. According to the coworker, he had just climbed the ladder to install one end of "T" bar. Employee #1 was climbing up his own ladder to install the other end of the same "T" bar. Employee #1's ladder was positioned approximately 2 to 3 feet away from the railing. As they both finished installing the "T" bar, the coworker observed as Employee #1 completed his end of the "T" bar, and as he stepped onto the floor, Employee #1 lost his balance and hit the railing. The top rail was 33-inches high and, according to management, the mid rail was not installed at the time of the accident. The momentum carried him over the railing. Employee #1 sustained pelvic fractures. Employee #1 was taken to Holy Cross Hospital in Mission Hills.
