Investigation Summary
On April 24, 2002, several employees were working on an outdoor storage area just as it started to pour with rain. The employees were instructed to cover the corrugated boxes with tarps to prevent the boxes from getting wet. The boxes were stacked in bundles and some of the bundles were piled as high as 20 ft. There was a lack of safe access and no work platform for employees to work on these high piles of stacked boxes. An employee was covering the last part of the bundled stack. The top of the boxes, as well as his shoes and clothing, were already wet. The employee slipped, lost his footing and began to fall. He fell approximately 13 ft, landing on other stacks, and then the ground. He was able to use his feet to slow him down and sustained a sprain. The employee was taken to USC Medical Center for treatment.
