Investigation Summary
At 8:30 a.m. on August 21, 2001, Employee #1, employed by Heafner Tire Group, suffered a torn muscle on his left bicep when he grabbed a vertical metal support of a storage rack after he became unbalanced while standing on the pallet and shelf of a storage rack at the third level inside a warehouse. Employee #1 wanted to pick a few spare tires from the shelf of the storage rack in Aisle Number 18 when the accident occurred. He fell about 2 feet after loosing his balance on the pallet he was working from. He was about 15 feet high on an order picker and tore a muscle on his left bicep due to his body weight when he fell. Employee #1 stated that he did not fall all the way down as he was tied off with a lanyard attached to the order picker or industrial truck he was using. Employee #1 was initially treated at an industrial medical clinic for a couple of hours and then underwent surgery two days later at another hospital.
