Employee injured after right foot run over by forklift

On October 9, 2001, Employee #1 and a coworker of Electra-Bond, Inc., had just finished wrapping some parts. Both Employee #1 and the coworker walked towards the forklift. Employee #1 approached the forklift to place the roll of saran looking wrapping while the coworker got on the forklift to drive it. The coworker immediately started the forklift and backed away and run over Employee #1's right foot. After the accident, Employee #1 went to hospital. According to the doctor's first report medical report, the employee suffered from contusion. He tore his ligament but did not fracture any bones. The medical diagnosis from the orthopedic doctor was "status post contusion/sprain, right ankle and with reflex system dystrophy."
