Investigation Summary
Employee #1, a laborer employed by a general commercial building contractor for 2 years, was working at an art school and gallery. The contractor was remodeling. He was in the process of removing a 7-inch wide, 10-foot tall metal door jamb leg from a concrete opening. After cutting the jamb leg into 4 pieces, he pried the bottom 3 pieces from the concrete. He used a 3 foot long, 1 inch thick metal pry bar with an arched, 8 inch long head, attached on the center. While he was attempting to remove the final and uppermost piece of jamb, standing on the third step of a 6 foot A-frame ladder, the head of the pry bar broke off at the weld point. Employee #1 lost his balance and fell approximately 3 feet, hitting the back of his head on the concrete floor. He lost consciousness momentarily, and suffered a concussion and possible skull fracture. He was transferred and admitted to a local hospital, where he spent 48 hours and was released.
