Investigation Summary
At about 8:30 a.m. on August 13, 1993, Employee #1 was working with his back to a blockout hole in a second floor deck pan. Another employee removed the plywood cover from the hole to finish around it. Employee #1 was floating the concrete and stepped backward through the previously covered hole. He fell 12 ft to concrete, fractured his right wrist and elbow, and was hospitalized for four days.
