Investigation Summary
On October 12, 1993, Employee #1, the senior production manager at Gekkeikan Sake, was helping the machine operator clean a rotary valve used to produce koji rice for sake. The rotary valve, used to transfer koji rice from the cooling conveyor system to the cultivating room tanks, had become clogged with sticky rice. The operator shut down the machine and cleaned out the rotary valve along with several other areas. When he had finished cleaning, the operator went to the control panel and pressed the start buttons without looking back at the machines. Not knowing the operator was starting the machine, Employee #1 had put his hand down into the rotary valve to clean out more rice. His finger was caught by the machine. First aid was applied by the Folsom City Fire Department before Employee #1 was transported to the UC Davis Medical Center in Sacramento, CA, where he was treated and released. No lockout procedures had been used. The rotary valve is a portable unit connected by a cord and plug without an ON/OFF switch. The outlet it is plugged into is controlled from a central panel. The rotary valve should have been unplugged from the outlet before it was cleaned.
