Employee's hand severely lacerated by plastic sheet

At about 9:00 a.m. on November 23, 1993, Employee #1 had cut a sheet of 1/8 in. plastic with a table saw. As he lifted the plastic sheet to remove it from the saw, the saw blade caught the edge of the sheet and spun it up under the guard. The plastic jumped the fence and spun into Employee #1, catching his left hand. Employee #1 suffered a severe laceration on his left hand between the thumb and forefinger.
