Investigation Summary

Investigation Nr: 170720379
Event: 04/27/1993
Employee injured in fall onto beet conveyor in pit

At approximately 9:10 a.m. on April 27, 1993, Employee #1 positioned the first of two 22-foot trailers in its dumping position to the north of the off-loading sugar beet pit. After he activated the handle to open the trailer gate, he slipped from the narrow access platform (no rails and no toe board present) into the pit and onto the fast moving conveyor belt. He was about 10 ft from the separator when the operator of the dirt/vegetation truck yelled "man on the conveyor." The emergency stop button was activated. First aid was administered by the Woodland Fire Department and Employee #1 was then transferred to UCD Medical Center Trauma Unit. He was discharged on May 1, 1993.
