Investigation Summary

Investigation Nr: 170716260
Event: 03/10/1993
Employee injured in collision between industrial trucks

At about 9:30 p.m. on March 10, 1993, Employee #1 of Albertson's was operating a motorized pallet jack at the Roseville Distribution Center, Roseville, CA. A forklift carrying a short load on its forks went through a clear plastic curtained doorway into a freezer. At the same time, Employee #1 came out of the ice cream room into the freezer riding the pallet jack through a curtained doorway. Employee #1 had his back turned to the doorway. The speed, although slow, was still too fast. The front corner of the forklift load collided with the rear corner of the pallet jack. The pallet and load struck and pinned the leg of Employee #1, the pallet jack operator, to the motor frame. First aid was applied and he was then transported to Roseville General Hospital, Roseville, with multiple breaks to the lower right leg and ankle.
