Investigation Summary
On August 10, 1995, Employee #1, a supervisor for R. J. Wood, Inc., and a production employee (machine operator) were troubleshooting a poly bag machine. While Employee #1 was holding the bag roll belt drive, a coworker moved another roller that activated a micro switch and caused the bag roll belt to move. Employee #1 suffered finger lacerations that required stitches, but he did not have any fractures or bone loss and is not expected to have any permanent disfigurement. Employee #1 was treated as an outpatient and returned to work in four days (including the weekend). The injuries were not serious as per 331.1(h). A serious violation, T8CCR 3314(a), was cited because of the exposure to possible serious injury. No lockout procedure was used although two disconnects were available on or near the machine. The employer has an operative written safety program that includes a comprehensive lockout/blockout procedure. An internal accident investigation was made.
