Employee's finger amputated by auger

At approximately 4:15 p.m. on June 26, 1995, Employee #1, of J.R. Wood, Inc., inserted his right ring finger inside a drain plug on a screw auger. The auger was energized and amputated the finger at the second joint. Employee #1 failed to stop, de-energize, and lock out the auger.
