Investigation Summary

Investigation Nr: 170608657
Event: 06/14/1995
Employee's thumb crushed when caught between pipe and tool

At approximately 11:00 a.m. on June 14, 1995, Employee #1, a 38-year-old laborer for Ameron Pipe, was working in a sandblasting unit. His job was to keep the 51 in. steel pipes close together as they moved from the sandblaster to the tar application unit. He was attempting to rotate a 51 in. steel pipe, using a special tool, when his thumb was caught between the end of the tool and an adjacent pipe. His thumb was crushed. The employer has redesigned the tool to prevent the user from placing his thumb over the end of the tool.
