Investigation Summary
On February 10, 1997, Employee #1 was using a robotic pick-up head, in manual mode, to place a bundle of telephone directory pages on a pallet. The bundle was not properly strapped, so once it was on the pallet Employee #1 raised the pick-up head to the 'stopped' position and bent over the pallet to realign it. As he was doing, this the cogged belt that controlled the pick-up head's vertical movement broke. The pick-up head fell, striking Employee #1 on the head, neck, shoulders, and back.
