Employee's fingers crushed by falling heavy machinery

Employee #1 and coworkers, of Advanced Industrial Services, a millwright contractor, were installing a chill roll that weighed approximately 13,000 lb. The machinery was on the forks of a large forklift. Employee #1 was kneeling to insert blocking under the load when the forks suddenly dropped and crushed his right fingers. Employee #1 was hospitalized for surgical revision of his right index finger to the middle joint.
