Employee injured in fall through chicken house roof

On January 13, 1997, Employee #1 was making a delivery of chicken feed to a customer. The feed tanks on one of the chicken houses had been enclosed at the bottom with plywood, making the lid lifter of feed tank #12 inoperable from outside the building. Delivery drivers are not allowed to enter chicken houses to avoid transmission of avian disease, so Employee #1 climbed a metal ladder adjacent to the corrugated aluminum roof. A 1 in. by 6 in. plank had been placed at the roof's edge. Employee #1 either stepped off or slipped off the plank onto an unsupported section of the roof and broke through. He fell approximately 12 ft onto a concrete floor and sustained head injuries that most likely will result in permanent blindness.
