Employee's finger crushed in depalletizer

On May 9, 1996, Employee #1, an experienced depalletizer operator, was trying to remove a slipsheet with her left hand while operating a control panel joystick to elevate stacked cans to an electromagnetic transfer device. The joystick was a constant contact control device that returned to a neutral center position when released. Employee #1's left index finger became caught and was crushed between the rising pallet of stacked cans and the electromagnetic transfer device. There was no indication of control failure. The standard operating procedure was to pull the slipsheets before elevating. Employee #1 had reached around an expanded metal guard designed to prevent inadvertent contact.
