Employee's arm injured when caught between trailer and dolly

On February 25, 1997, Employee #1, a lumber handler for a lumber company, was hitching a heavy duty CL6 Caterpillar forklift to a wood chip trailer van dolly tongue. He was adjusting the elevation of the tongue eye on the dolly to accommodate the rear-mounted hook on the forklift when the unbalanced dolly tongue rotated upward. Employee #1 suffered a serious laceration to his right forearm when it became caught between the tongue and the underside of the trailer van. The vertical height adjustment post, wheel, and crank were between the dolly and van, exposing Employee #1 to this poor design.
