Investigation Summary
AT APPROXIMATELY 3:45 ON AUGUST 15, 1993, EMPLOYEE #1 ATTEMPTED TO PLACE A SMALL GEAR ON THE SHAFT OF A HYDRAULIC MOTOR WHILE IT WAS MOVING. WHEN HE DID SO, HIS RIGHT THUMB WAS CARRIED THROUGH THE MESH POINT BETWEEN THE GEAR HE WAS HOLDING AND THE LARGE BULL GEAR OF THE WHEEL LINE IRRIGATOR. THERE WAS NO GUARD OVER THE GEAR MESH POINT (NIP POINT). THE EMPLOYER HELD A SAFETY MEETING COVERING THIS ACCIDENT.
