Investigation Summary

Investigation Nr: 170632582
Event: 09/18/1996
Employee injured in fall while climbing ladder

Employee #1, a foreman HVAC installer with Hanley Heating and Air, was climbing a ladder to reach an elevated location when he fell 15 ft to the floor. He was transported to the hospital, where he remained for over 24 hours. Although the cause of the accident was not determined, the employer was cited for lack of fall protection during the installation of overhead duct work. The employer was also cited for unguarded energized parts because of the 220 volt uninsulated wires that were in close proximity to Employee #1 when he fell.
