Investigation Summary
At approximately 12:00 noon on August 27, 1996, Employee #1, of the Hoffmeyer Company, was applying rubber bonding to a pulley drum. When he reached under the drum to apply pressure to its deep areas, the drum spun off its stands and fell on him. Employee #1 sustained serious facial and head injuries and was taken to St. Francis Hospital. He was hospitalized for 25 days and then treated as an outpatient.
