Investigation Summary
At approximately 7:30 a.m. on December 16, 1998, Employee #1, of the City of Hope Medical Center, was using a table saw to cut a piece of wood in the carpenter's shop when his right hand struck the running blade. He amputated the tip of his middle finger and lacerated several other fingers. The employer stated that a guard was available, but that Employee #1 had chosen not to use it. The saw's operating/instruction manual was not available for employees to read and follow.
