Investigation Summary
At approximately 7:45 a.m. on March 5, 1996, Employee #1 was cleaning a Case printing machine. He was trying to dislodge a piece of material from the rollers when his left hand was pulled into the machine. The tip of his index finger was partially amputated, and his middle finger was lacerated. He was taken to the Western Hand Center, where he was hospitalized for two days and then treated as an outpatient. The rollers were operational when the accident occurred and the machine was unguarded.
