Employee's hand injured when caught in laminator rollers

Employee #1, of Bay Cities Container corporation, Pico Rivera, CA, had been supplied with a 10 in. extension tool to clean the glue rollers and banks on an Automation 7780 laminator. He was working with the machine in the 'off' position, but felt it would be quicker and easier to energize the laminator for placing bands on the in-running rollers. The extension tool struck the moving rollers and Employee #1's right hand was pulled into the rollers' nip point. Employee #1's hand was crushed, and he sustained soft tissue injuries and open avulsion fractures. The causal factors of this accident were failure to follow the lockout/tagout procedure, and the fact that the rollers were unguarded.
