Investigation Summary
At approximately 10:05 a.m. on January 21, 1994, Employee #1, of Ryko Products, Inc., was adjusting a stuck butterfly valve to re-open in a high intensity mixture. He inserted his right hand into the danger zone without shutting off the equipment. The air valve closed on his hand, amputating four fingers. He was taken to Master Facility Industrial Clinic and later to Long Beach Memorial Hospital and was hospitalized for treatment.
