Employee's fingers amputated in wood shaper

At approximately 8:30 a.m. on April 19, 1996, Employee #1, of The Wood Designer, Inc., Pomona, CA, was using a Holzher shaper 550/750, with a double spindle, to make frame molds for mirrors. He had made about 200 frame molds that morning and had frames remaining that had to be run through the shaper twice. Employee #1 ran a frame through the shaper once; as he tried to run it through again, the guard on the shaper fell apart. He then tried to run the part without the guard. His right hand contacted the exposed blade, amputating his ring and middle fingers. The employer was cited for a serious violation of T8CCR 4318.1(a). Other violations were also found, and the employer was cited accordingly.
