Investigation Summary
At approximately 10:00 a.m. on April 8, 1994, Employee #1 was using a table saw with an unguarded saw blade to cut a 4 ft by 8 ft masonite sheet. After he had finished a cut, his left hand was pulled across the revolving saw blade. Employee #1's left index and middle finger made contact with the saw blade and the undersides of his index and ring fingers were lacerated below the knuckle. He was transported by paramedics to Amador Sutter Hospital in Jackson, CA, where he was admitted for five days.
