Employee's fingertip amputated by shaper knife

Employee #1 was cutting the outside detail of a drawer front with a wood shaper. The shaper was correctly guarded and, as an added precaution, Employee #1 was also using a push stick to move the drawer through the shaper blades. Employee #1 pushed with his right hand and guided the piece with his left. His left hand somehow slipped and his left index finger slid along the edge of the drawer and into the turning shaper knives. About 1 in. of Employee #1's left index fingertip was amputated.
