FINGERS AMPUTATED-WIPING DRIPPAGE AROUND TIP DEPOSITOR

At approximately 8:30 a.m. on September 17, 1997, Employee #1 was working at the KWA Line as a forming operator. He attempted to wipe dripping chocolate from the area around the tip depositor using a rag. The machine was running at the time. The rag became caught at the edge of the mould and lift table. His middle, right hand finger became caught in the machine. As a result, the tip of the middle finger of his right hand needed to be amputated. Employee #1 was treated and released from Oak Valley Hospital.
