Investigation Summary
At approximately 2:00 p.m. on September 26, 1996, Employee #1 was on the third rung from the top of an 8 ft fiberglass portable ladder, running electrical conduit. The conduit had been anchored to the wall by Employee #1, who was working alone. As he was applying pressure with an off-set pipe bender to create a slight bend, he lost his balance and fell from the ladder to the concrete floor. Employee #1 sustained injuries, including dislocation of both elbows and fractures to both hands, for which he was hospitalized.
