Investigation Summary
An electric utility supervisor was working with a crew of eight electricians. They were installing a 225-kilogram magnetic generator in an 8.2-meter-long by 3.4-meter-diameter steel cylinder that had a 710- to 760-millimeter opening on its west end. The cylinder was located parallel to the axis of a pit in a parapet retaining wall. The pit was 15.2 meters long, 2.4 meters deep, and 2.7 meters in diameter. The wall was 530 millimeters high. The work platform over the pit at the west end of the cylinder consisted of a 1.2-meter by 2.4-meter sheet of 19-millimeter-thick plywood resting on two 280-millimeter-wide planks that could be extended in length from 305 to 4165 millimeters. The platform design left an opening of about 150 millimeters by 610 millimeters on each end. The employees accessed the platform over two 150-millimeter-diameter pipes that were about 530 millimeters high, extending 530 millimeters from the north retaining wall. The employer had provided no steps to reach the platform and had not equipped it with guardrails. The supervisor stepped in an opening on the north end of the work platform and fell, striking the right side of his face and head on fixed equipment. He suffered facial fractures and lacerations and was hospitalized for his injuries.
