Investigation Summary

Investigation Nr: 170614044
Event: 06/22/1994
BURN - ELECTRICAL FAULT IN PANELBOARD

An electrician in a saw mill had run a temporary cord from a motor for a set of outfeed rolls to a 3000-volt panelboard. As the electrician was replacing fuses in the panelboard, the last fuse contacted a holder for one of the other fuses. This created a phase-to-phase electrical fault. The ensuing electric arc burned the employee's hands, forearms, and face, and the right side of his body. The electrician stated that he had opened a switch and tested it with a tester to be sure the panelboard was deenergized before he started working on it. He did not know how it had become energized. The employee was hospitalized for his injuries.
