Millwright's fingers amputated in chain conveyor

Employee #1 was employed as the floor millwright at a sawmill. The hula trim waste conveyor jammed and he de-energized the conveyor's power source. He shut off associated conveyors and cleared the jam, then turned all the conveyors back on. A chunk of wood went into the tail pulley and the conveyor stopped again. This is where the previous jam had occurred and he assumed that the conveyor's electrical circuit breaker had tripped. Employee #1 then reached in to pull loose the chunk of wood. The conveyor started and his arm was pulled into the conveyor and around the tail drum. Several fingers were amputated. The hula trim waste conveyor can be operated from other locations and it had been stopped and then started from one of these locations. The conveyor has a box 1-10 chain and is driven by a 7.5 horsepower motor.
