Investigation Summary
Employee #1, a millwright at a single manufacturing plant, heard an unusual noise in the #2 former area and climbed the guardrails and machinery frame supports to see if he could find what was causing it. He had a flashlight in one hand and a container of lubricant in the other. According to Employee #1, his foot slipped and when he reached out to steady himself, his little finger was run over by a wheel on the #2 former. The fingertip was crushed and later removed at the hospital.
