Investigation Summary
Employee #1, a laborer for Roy E. Ladd, Inc., a construction company, was loading a 1,000 gallon fuel tank onto a flatbed truck. Employee #1 stood on the forks of a forklift truck to hook a chain to the tank and gripped the forklift mast for balance. The second stage of the mast caught and amputated the fingers of his right hand as the mast was being lowered.
